To qualify for a group policy, you need at least two enrolled employees, or 35% of eligible employees—whichever is greater.
No, employer contribution is not mandatory. However, if you choose to contribute, we can reduce the premium rates.
Coverage begins on the first day of the month. If we receive all the necessary paperwork by the 15th of the current month, we can back-date the start date to the first of that current month. For example, if you send in paperwork on August 13, the group can have coverage starting from August 1.
To initiate a group policy, we require a completed group application and an enrollment form for each employee enrolling. Additionally, if you prefer to pay via ACH, we will need an ACH form.
No, payments must be made by the group rather than directly by the employees.
