Becoming appointed as a producer for Delta Dental of Idaho is easy. If you are a producer
licensed with the Idaho Department of Insurance and would like to be appointed to do business
with Delta Dental of Idaho, please mail or fax a copy of your State of Idaho producer license
and Tax ID number (for agency) or Social Security number (for individual).

Upon receipt of these items, we will forward them to the Idaho Department of Insurance for
appointment. Please allow at least 10 business days for the Department of Insurance’s Licensing
to return the appointment certificate. By state law, you must be appointed with Delta Dental of
Idaho before you can receive a quote for any of your clients. For further information, please

Groups with 3-99 Employees
We’ve made it easy to receive a quote for a Group with 3-99 employees. If you are registered to use the
secure Producer Log-In section of our website,
simply click Get A Quote to begin. If you are not

Groups with 100+ Employees
We will customize a plan for large employers that can save on dental claims costs and provide a
provide a quality dental plan that encourages prevention of dental disease.

Manual Enrollment
- Complete a Group Application form.
- Provide Employee Enrollment forms, or provide an electronic enrollment file.
- Include a check for one month’s premium payable to: Delta Dental of Idaho.
- Include a current medical group billing statement (for Groups with 3-9 employees).
- Submit the completed items to Delta Dental of Idaho by the 15th of the month for
coverage to begin that same month (for example, January effective date must be received
by January 15th).
Paperless Enrollment
Groups can enroll employees and pay their monthly premium online. If you generated a quote online for
a sold group, that information you included in the quote will automatically generate into a
group application. Click on Saved Quotes
to access your archived proposals. Then select Initiate Online Application under that group’s quote.
You will then be emailed the information which you can provide to the new group so they can login and
complete the online application process. Once the group finalizes the group application, they can enroll
employees online.
